The pad thai was excellent. Six hours later you are on the bathroom floor of a hotel room wondering whether this is the kind of thing you call someone about. Sudden vomiting and cramps after a meal is the single most common reason travellers see a doctor in Phuket. Be careful with anti-diarrhoeal medication

Loperamide sits in most travel kits and is the wrong choice in several common situations.

Do not take it if you have a fever, or blood or mucus in the stool. Those suggest an invasive bacterial infection, and slowing the gut keeps the organism and its toxin inside you for longer, which can turn a bad few days into something considerably worse.

When to come in

  • Vomiting that has stopped you keeping any fluid down for more than a few hours

  • Blood in the stool or in vomit
  • Fever above 38.5C
  • Severe pain concentrated in one spot rather than general cramps
  • Symptoms continuing beyond three days
  • Any of the above in a young child or an older traveller

Severe pain fixed in the lower right abdomen deserves same-day assessment, because appendicitis is regularly mistaken for food poisoning in the first day.

Antibiotics are not automatic and are frequently unhelpful — most food poisoning is self-limiting, and antibiotics do nothing at all for viral or toxin-mediated illness. They are used when the clinical picture points to a bacterial cause likely to respond.

Street food is not automatically the culprit and avoiding it entirely is both unnecessary and a poor trade. A busy stall cooking to order in front of you is frequently safer than a hotel buffet sitting under a warming lamp for two hours.

The things that actually raise risk are food kept warm rather than hot, anything reheated more than once, raw or lightly cooked shellfish, salads rinsed in tap water, ice from an unknown source, and fruit peeled by someone else hours earlier.

Practical rules: eat where there is turnover, prefer food cooked in front of you and served hot, peel your own fruit, and use bottled or filtered water — including for brushing teeth if your stomach is already upset. Hand hygiene before eating prevents more cases than any amount of menu avoidance, and is the part most people skip.
