Rabies is effectively always fatal once symptoms appear, and it is almost entirely preventable if treatment starts in time. Wash the wound first — this matters more than anything
Before you travel to us, wash the bite or scratch under running water with soap for a full fifteen minutes. Fifteen minutes feels absurd when you are standing at a sink. A licked graze counts if the skin was already broken. So does a scratch that barely bled. The animal appearing perfectly healthy proves nothing at all — an infectious animal can look and behave normally for days before it sickens. Do not wait to see how the animal gets on.
What the course involves
If you have never been vaccinated against rabies, you need a series of injections into the arm over roughly a month — typically day 0, 3, 7 and then a final dose somewhere between day 14 and 28 depending on the schedule used.
For deeper or higher-risk wounds, rabies immunoglobulin is also infiltrated around the wound itself at the start. That provides immediate passive protection while your own immune response is still building over the first week or two. It is the expensive part of the treatment and the part that is not always easy to source, which is one reason to start at a clinic that stocks it rather than shopping around.
If you were vaccinated before travelling, you need only two booster doses and no immunoglobulin at all.
Finishing the course after you fly home
Most travellers leave Phuket part-way through the schedule and that is entirely workable, provided you actually finish it.

Do not abandon the course because you feel completely fine. There are no symptoms whatsoever during the incubation period, which can run for weeks or months, and by the time there are symptoms it is far too late for treatment to do anything.
The other risks from an animal bite
Rabies takes the attention, and ordinary bacterial infection is what actually sends most people back to us. Cat bites are particularly treacherous: fine sharp teeth make deep narrow punctures that seal over within hours, trapping bacteria in the tissue underneath. A cat bite to the hand can become a serious infection inside twenty-four hours.

Getting vaccinated before you travel
Pre-exposure rabies vaccination is worth considering if you are staying for months, cycling or hiking in rural areas, working with animals, or travelling with young children — children are bitten more often, are bitten on the face and hands more often, and are much less likely to report a small scratch.
It does not make you immune. What it does is remove the need for immunoglobulin after an exposure and reduce the course to two doses, which is a substantial practical advantage somewhere immunoglobulin may be hard to obtain quickly. How soon do I need the first injection?
The same day if at all possible. There is no fixed cut-off after which it becomes pointless — it is still worth starting late — but the sooner it begins, the better it works.
How much does the rabies course cost?
Each dose is priced individually, and immunoglobulin, where it is needed, costs considerably more than the vaccine itself. You are told the full cost of the course at the first visit, including which doses you pay for here and which abroad.
The animal looked healthy. Do I still need it?
Yes. An animal can transmit rabies before it appears unwell, and in practice you will never see that dog again to check. The treatment is safe and the disease is not survivable.
Can I get the injections at my hotel?
Yes, a doctor hotel visit can give scheduled doses in your room. The first visit is better at the clinic so the wound can be assessed and irrigated properly.
I had the pre-travel vaccine. Does that cover me?
It changes the treatment rather than removing it. You still need two booster doses after an exposure, but you do not need immunoglobulin — which is the expensive and harder-to-source part.
Are there side effects?
A sore arm for a day or two is normal, and some people feel mildly off-colour that evening. Serious reactions are rare. There is no medical reason to avoid the course, and no alternative to it if you have been exposed.
