You counted eleven bites on one ankle after an evening at a beachfront bar, and one of them has swollen to the size of a coin and feels hot to touch. Most bites here are harmless and maddening. Infection develops later, usually after two or three days, and gets worse rather than better. The signs are increasing pain rather than itch, spreading redness, warmth, pus, and sometimes fever.

Scratching is what turns an itchy bite into an infected one, and in this climate that transition happens far faster than it would at home.

Ticks need removing properly

Grip the tick as close to the skin as possible with fine tweezers and pull straight out with steady pressure.

Do not twist, and do not burn it, smother it in oil, or apply alcohol first. All of those make the tick regurgitate into the wound, which is precisely the opposite of what you want.

Wash the area afterwards and watch it for two weeks. Come in if a spreading circular rash develops, or if you get fever, headache or joint aches. If the head is left behind, or you are not confident removing it, we will do it.

What actually relieves the itch

Antihistamine tablets work considerably better than creams when you have multiple bites, and are most useful taken at night when itching reliably peaks.

A steroid cream settles individual inflamed bites. Cool compresses help, and so does keeping the skin cool generally — heat makes everything itch more.

Do not scratch. Keeping nails short and covering bites with a light dressing overnight breaks the cycle more effectively than willpower. Avoid the strong topical anaesthetic sprays sold in tourist shops, which frequently cause a contact reaction on top of the bite.

When to be seen

Come in if there is:

  • Spreading redness, or a red streak running up the limb
  • A bite that is increasingly painful after day two
  • Pus, or a bite that has become an abscess
  • Fever after being bitten
  • A centipede or scorpion sting, which can be severely painful
  • Widespread hives, facial swelling or any breathing difficulty

Breathing difficulty or facial swelling after a sting is an allergic reaction and is an emergency — call 1669. A bite that becomes a hot, spreading red area is cellulitis and needs antibiotics the same day.

Not getting bitten in the first place

Repellent is the main thing, and the details matter more than the brand. DEET at 20 to 50 percent or picaridin at 20 percent both work well. Apply it to exposed skin after sunscreen, not before, and reapply after swimming and heavy sweating.

Timing is where people go wrong. The mosquito that carries dengue bites during the day, so evening-only application misses the actual risk. Sandflies are worst at dusk and bite around the ankles, which is why bites cluster there after sunset drinks on the beach.

Air conditioning and a ceiling fan both help considerably, since mosquitoes are weak fliers and avoid moving air. Empty standing water near your room — plant saucers, buckets, pooled water on balconies — because these mosquitoes breed in remarkably small volumes and rarely travel far. If you are getting rows of bites overnight and nobody else in the room is, check the mattress seams for bed bugs rather than reaching for more repellent.

Why some people get bitten far more than others

It is not imagination, and it is not sweetness of blood. Mosquitoes locate people by carbon dioxide, body heat and skin chemistry, and the differences between individuals are substantial and largely fixed.

Larger people and pregnant women exhale more carbon dioxide and run warmer, so they are found first. Exercise raises both body temperature and lactic acid on the skin, which is why a run at dusk is efficiently punished. Beer measurably increases attractiveness to mosquitoes. Dark clothing is easier for them to see against a background.

None of that is changeable, which is why repellent matters more for some people than others rather than being optional for everyone.
